Where can I find the personal plan income tax claim statement and premium statement that I need for my taxes?

If you have a personal/individual Medavie Blue Cross plan (a plan you’ve purchased from Medavie Blue Cross directly not through an employer) your income tax claim statement and premium statement can be located online, the steps for each are outlined below. If you are unable to locate it using these steps, you can also access it by contacting our customer service team. If you have a group plan (a plan provided by your employer or another organization) follow the link for instructions on where to find your Group plan income tax claim statement.

Accessing your Income Tax Claim Statements Individual:

Go to www.medaviebc.ca

Click Login (top right-hand corner of the page)
Click Plan Members
Click Log in
Put in your credentials (login name which is your email address and password - if you use the mobile app your login name is the same as for the website
Click on Claims (located along the top of the page on the left-hand side)
Click on View Claims History in the drop-down menu
Go to the middle of the page. In the search history, put the dates you are looking for on the Start Date and End Date example 01/01/2022 – 31/12/2022
In the “Who it is for” section there is a drop-down, select the participant you would like to run the statement for
In the Benefit Category leave it as all
Click the blue Search button

Once the page loads scroll to the bottom of the page and click download. This will populate a PDF, once you open this file you can print or save it as a PDF.

Accessing your Income Tax Premium Statements Individual:

Go to www.medaviebc.ca

Click Login (top right-hand corner of the page)
Click Plan Members
Click Log in
Put in your credentials (login name which is your email address and password - if you use the mobile app your login name is the same as for the website
Select My Account ( top right-hand corner of the page)
Select Manage Your Account within the drop-down menu
Select Review and print your premium statements

This will populate a PDF, once you open this file you can print or save it as a PDF.
